The fire began in a subbasement when nitrocellulose x-ray film caught fire, possibly because of a faulty light bulb. The fire, which generated poisonous gas that made its way via the building's ducts to every room in the structure, claimed 123 lives and caused two explosions. As a direct result of the fire, Cleveland firefighters were issued gas masks.
