Same old story

No sprinklers, insufficient exits, flammable finishes, and no code enforcement. All conspired to devastating effect when the band set off pyrotechnics on stage. The fourth deadliest U.S. nightclub fire caused 100 deaths and more than twice as many injuries.

As a result of the fire, Rhode Island vowed to become the safest state and swiftly enacted changes to the fire and building codes to ensure that the tragedy would not be repeated.
